Making a claim

To make a claim on your insurance, contact us to check your eligibility and obtain a link to complete your claim online or obtain the necessary forms and relevant documents.

How do I make an insurance claim?

Insurance in your super is there for when you need it most. If you need to make a claim, call us on 1800 000 086 and we can help guide you through the process. 

If you wish, we can also initiate an online claim for you and email you the link so you can complete the claim at home. Once your claim has been lodged using the online claim portal, you will be able to log in and track its progress.

Insurance cover through ANZ Staff Super is provided through a Group Life Policy ANZ Staff Super holds with Zurich (the Insurer). When you make a claim, you will see Zurich’s logo on many of the documents.

As your claim progresses, Zurich’s team of insurance claim specialists will follow up on a regular basis and provide support for any questions you may have.

What do I need to complete a claim?

The information you will need to provide to make a claim will depend on the type of claim you are lodging.  For example, if you are looking to submit a Total and Permanent Disablement claim, you will need:

  • Details of your injury or illness
  • Details of your treating doctors
  • Details about your work status and the date you last worked
  • Reports from your treating doctors.

When you call us, we’ll be able to let you know what you’ll need to provide.

If you prefer to use a paper form, we can direct you to the correct forms on the website or mail them to you.   

Death claim

If you are a family member or legal personal representative of  an ANZ Staff Super member who has passed away, please call us on 1800 000 086. We will initiate a death benefit claim and help you through the next steps.


Insurance cover is provided by Zurich Australia Limited ABN 92 000 010 195 (the “Insurer”) and subject to the terms and conditions of the insurance policy issued to ANZ Staff Superannuation (Australia) Pty Limited ABN 92 006 680 664 AFSL 238268 RSEL L0000543 (the Trustee of ANZ Staff Super) by the Insurer (the “Policy”). You should read the Product Disclosure Statement (PDS) for your membership section for a summary of the terms and conditions of the Policy. Your Section application will be assessed by the Insurer and ANZ Staff Super will advise you of the outcome in writing.
